Details:

We are seeking a social worker for the hospital discharge team in the Wythenshawe area of Manchester. The role involves assessing citizens for care act requirements and conducting timely reviews of those being discharged from hospital. Essential experience includes completing care act assessments, mental capacity assessments, making best interest decisions, and safeguarding.

The social worker will assist in completing reviews within a few weeks after discharge, evaluate care packages arranged by hospital teams, and assess for long-term support before transitioning individuals to community teams.

The role involves providing direct support to service users and their families, ensuring compliance with statutory responsibilities, policies, and procedures to achieve positive outcomes.

The successful candidate will collaborate with City Council colleagues, partner agencies, and stakeholders to manage present and future risks and to develop person-centred care plans that promote independence.

Additional responsibilities include supervising and developing students and trainees, and contributing to staff development.

Key Responsibilities:

  • Develop and maintain relationships supporting service users and families, making complex decisions to facilitate access to improved opportunities.
  • Assess and plan for individual needs in collaboration with partner agencies, ensuring positive outcomes for Manchester residents.
  • Develop effective solutions to improve service delivery and engagement, focusing on quality outcomes.
