Adult's Social WorkerTeam: Front Door & Urgent Community ResponseLocation: StockportRate: £30
A temporary post has arisen for an experienced Social Worker.
This role is to work alongside the Front Door Team and the Urgent Community Response Team.
Key responsibilities include:
– Working alongside Health colleagues to reduce the need for hospital attendance
– Completing visits as and when required, including same day response.
– Completing Care Act and short term assessments, with a focus on reducing need and promoting a
persons independence in their own home
– To provide a Social Work response and expertise within a multi-disciplinary team
The role is office based, a clean driving licence and transport are required
