Role Overview

As a member of a multi-disciplinary, multi-professional team, the Social Worker will effectively manage an agreed caseload of complex cases in accordance with Blackburn with Darwen Borough Council Policies and statutory legislation. Responsibilities include:

1. Assessing and establishing needs, identifying options/solutions, or signposting to colleagues and other services to meet those needs.
2. Undertaking social care assessments, s.42 safeguarding enquiries, initial reviews, annual reviews, completing capacity assessments, and best interest meetings.

Specific Responsibilities

For Adult Community Social Care Posts:

1. Completing short term care reviews following discharge to assess placements.
2. Supporting individuals to return home in a safe and timely manner.

For Hospital Social Worker Position:

The role focuses on the Home First approach to discharge, supporting with triaging referrals with the Homefirst and Reablement team colleagues, undertaking a duty role, and working with individuals throughout their journey from hospital or rehabilitation.
