The Role

As Care Co-ordinator, you will be responsible for overseeing the day-to-day coordination of care packages, staffing schedules and service delivery. Working closely with management, care staff, service users and external professionals, you will ensure that services are delivered safely, consistently and in line with regulatory requirements. Key Responsibilities

  • Planning and maintaining staff rotas to ensure all care visits are covered effectively
  • Matching care staff to service users to promote continuity and quality of care
  • Responding quickly to staffing shortages, sickness and emergency cover requirements
  • Supporting assessments, care plans, support plans and risk assessments
  • Monitoring service quality through spot checks, observations and supervisions
  • Supporting safeguarding procedures and ensuring incidents are appropriately reported and managed
  • Maintaining accurate records and compliance documentation
  • Supporting staff induction, training and ongoing development
  • Liaising with families, social workers, healthcare professionals and commissioners
  • Assisting with the mobilisation of new care packages and service reviews
  • Participating in an on-call rota to support operational requirements

About You

To be successful in this role you will have:

  • Previous experience within supported living, domiciliary care or community-based care services
  • Experience coordinating care services, scheduling staff or working in a senior care role
  • A good understanding of person-centred care and safeguarding practices
  • Strong organisational and problem-solving abilities
  • Excellent communication and interpersonal skills
  • Confidence working with care management and rostering systems
  • A Level 3 Diploma in Health & Social Care, or be working towards this qualification
  • A full UK driving licence and access to your own vehicle

Desirable Experience

  • Experience supporting individuals with learning disabilities and/or autism
  • Knowledge of the Mental Capacity Act and DoLS
  • Experience participating in an on-call rota
  • Knowledge of CQC standards and inspection requirements
  • Experience contributing to quality assurance and compliance activities
  • Local knowledge of Hemel Hempstead and the wider Hertfordshire area
