We are seeking a passionate and dedicated Social Worker to join the Positive Futures Team, supporting children and young people aged 16β18 as they prepare for adulthood.

What You Will Do
- Direct Support: Provide consistent support and guidance to children and young people aged 16β18 as they prepare for adulthood.
- Relationship Building: Build meaningful and positive relationships with children and young people, ensuring their needs remain central to your practice.
- Case Management: Manage a caseload of approximately 15β17 children and young people while maintaining high-quality social work practice.
- Regular Visits: Complete regular visits to children and young people, including those placed outside the local area.
- Face-to-Face Engagement: Maintain regular face-to-face engagement with children and young people in the community and at their placements.
- Case Recording: Maintain timely, accurate and high-quality case records.
- Team Working: Attend duty days and team meetings as part of the hybrid working arrangement and contribute effectively to team collaboration.
Requirements
- Qualified Social Worker
- Experience working with children in care
- Strong commitment to relationship-based practice
- Good organizational skills
- Ability to maintain timely, accurate and high-quality case recording
- Ability and willingness to travel as required for visits
