Working hours: 40 hours per week, Mon-Fri (between the hours of 08:00-18:30 on a rota basis)

Job Summary

As a Remote Advanced Clinical Practitioner, you will work autonomously to provide advanced clinical care to patients across all Archvale practices. You will use your expertise to deliver high-quality care through remote consultations, triage, adhering to clinical guidelines and contributing to the effective management of the practice.

Responsibilities

  • Delivery of highly effective, patient-centered care through remote synchronous and asynchronous consultations.
  • Performing remote triage and prioritising patient needs effectively.
  • Management of acute and long-term conditions, including medication reviews.
  • Processing administration tasks promptly, including referrals, repeat prescription requests, and other related documentation.
  • Maintaining accurate, up-to-date clinical records in line with best practices and policy.
  • Supporting the achievement of practice targets, including QOF indicators.
  • Participating in audits, significant event reviews, and quality improvement initiatives.
  • Collaborating with multidisciplinary teams to ensure coordinated and continuous care.
  • Contributing to the development and implementation of clinical protocols and guidelines.
  • Participating in appraisals, revalidation processes, and continuous professional development.
  • Promoting a culture of safety, learning, and continuous improvement within the practice.

Learning and Development

The effective use of training and development is fundamental in ensuring that all staff are equipped with the appropriate skills, knowledge, attitude, and competencies to perform their role. All staff will be required to partake in and complete mandatory training as directed by the training coordinator. Staff will also be permitted (subject to approval) to undertake external training courses which will enhance their knowledge and skills, progress their career, and ultimately, enable them to improve processes and service delivery.

Collaborative Working

All staff are to recognise the significance of collaborative working. Teamwork is essential in multidisciplinary environments. Effective communication is essential and all staff must ensure they communicate in a manner that enables the sharing of information in an appropriate manner.

Service Delivery

Staff at the surgery must adhere to the information contained with practice policies and regional directives, ensuring protocols are adhered to at all times. Staff will be given detailed information during the induction process regarding policy and procedure.

Security

The security of Archvale & the practices is the responsibility of all personnel. Staff must ensure they remain vigilant at all times and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.

Essential Criteria

  • Registered healthcare professional (e.g., Advanced Nurse Practitioner, Paramedic Practitioner, Pharmacist) with an Advanced Clinical Practice qualification.
  • Independent prescribing qualification.
  • Minimum of 3 years’ experience working as an Advanced Clinical Practitioner in a primary care setting.
