Key responsibilities:

  • Deliver high-quality community nursing care to patients in their homes and community settings.
  • Carry out assessments, care planning and clinical interventions as a Band 5 nurse.
  • Manage and prioritise a caseload of community patients.
  • Administer medications and carry out clinical procedures in line with NMC standards.
  • Work collaboratively with the wider neighbourhood MDT including GPs, social care and therapy teams.
  • Complete accurate and timely clinical documentation.
  • Escalate concerns regarding patient deterioration appropriately.
  • Contribute to the safe and effective running of the community nursing service.

Essential:

  • Registered Nurse with active NMC pin.
  • Experience in community or district nursing (desirable but not essential).
  • Ability to manage own caseload and work autonomously.
  • Full UK driving licence and access to a vehicle.
  • Good communication and documentation skills.
  • Enhanced DBS check required.
