Applefields School is seeking a Social Care Coordinator to join our supportive team in the Yorkshire Dales. The successful candidate will work from home and office to complete assessments and support plans for individuals aged 18 to 75.
Applicants should understand the Care Act 2014 and demonstrate professionalism. A Health and Social Care NVQ level 3 is preferred or candidates must be willing to obtain it.
