We're recruiting Social Workers to work as part of our dynamic Intermediate Care team. What You’ll Do:
- Work directly with people in their own homes to prevent hospital admissions, offering timely, tailored support when it matters most.
- Discharge people into Intermediate Care Beds when needed, ensuring they receive the right level of care to recover and return home safely.
- Lead short-term, high-impact interventions with individuals, families, and carers—developing personalised care plans that support recovery and independence.
- Collaborate across health, social care, and voluntary sectors to deliver truly integrated care.
- Support safeguarding work and uphold national standards for care and support eligibility.
About the Role
As a Social Worker, forming collaborative partnerships will be key. You will work closely with a variety of teams and agencies, including health colleagues, stronger communities and prevention teams, voluntary and independent sector, promoting health and wellbeing and preventing, reducing, and delaying need, in line with the Care Act. Through strength-based assessments and interventions, you will support adults with care and support needs and their carers to achieve their desired outcomes and wellbeing. To achieve this, you will be experienced in developing personalised care and support plans that represent best value and in determining eligibility for adults with care and support needs against the national standards, ensuring they are safe from harm. You will also have a good working knowledge of current adult social care legislation and application of the legislative principles, including the Care Act 2014.
