Are you passionate about helping people live independently and confidently? Join our Health and Adult Services team as a Senior Intermediate Care Worker and play a vital role in supporting adults and their carers to regain skills, improve wellbeing, and achieve personal goals all within their own homes.
About the Role
You’ll support the Intermediate Care Team Leader to coordinate a small team and deliver short-term, person-centred interventions that promote independence and recovery. This is a hands-on role where you’ll work with people who have the potential to improve their independence, including those with physical disabilities, learning disabilities, autism, sensory impairments, and low-level mental health needs.
What You’ll Do
- Carry out simple assessments and arrange equipment, adaptations, and assistive technology.
- Act as Lead Worker for cases, setting goals and monitoring progress.
- Provide practical support, including fitting equipment and assisting with personal care.
- Promote independence, wellbeing, and access to community resources.
- Work collaboratively with NHS, voluntary sector, and other partners to deliver preventative care.

About You
- Experience in adult social care or intermediate care services.
- Strong communication and organisational skills.
- Ability to work independently and as part of a team.
- Flexible to work across a 7-day service and travel within the community.
