Overview

As a Complex Care Coordinator, you will play a vital role in ensuring our clients receive safe, high-quality care in their own homes. Based at your local Apollo Home Healthcare office in Heywood, you will travel across the Northwest region to coordinate complex care packages, manage staffing schedules, and ensure seamless service delivery. A Full UK Driving Licence and access to your own vehicle are required due to travel across the region.

Responsibilities

  • Act as the first point of contact for our clients, their families, and healthcare professionals.
  • Effectively manage rotas for care teams, responding efficiently to day-to-day changes.
  • Monitor care delivery to ensure it meets individual care plans and quality standards.
  • Participate in multi-agency meetings with Integrated Care Boards (ICBs), advocating for client needs and contributing to care planning decisions.
  • Coordinate complex care packages and ensure service delivery supports client wellbeing and safety.
  • Collaborate with colleagues across care teams to support staffing and care delivery in the Northwest region.
  • Demonstrate strong organisational skills with the ability to manage multiple care packages and respond quickly to changing priorities.
  • Make decisions confidently, think on your feet, and stay calm and professional while solving problems.
  • Communicate clearly, adapting style to suit families, professionals, and external partners.
  • Negotiate and influence, particularly when managing rotas or balancing client needs.
  • Build and maintain strong working relationships as a proactive team player.
  • Gain specialist knowledge and leadership skills by working closely with healthcare professionals, case managers, and commissioners in complex care packages.

Requirements

  • Previous experience in care coordination, rota management, workforce planning, field supervision, care management, recruitment coordination, or a similar operational role within health or social care.
  • Understanding of person-centred care and planning.
  • Strong organisational skills with the ability to manage multiple care packages and respond quickly to changing priorities.
  • Confident in decision-making with the ability to think on your feet while staying calm and professional.
  • Clear communication skills, with the ability to adapt style for different audiences.
  • Comfortable negotiating and influencing, particularly when scheduling care and balancing client needs.
