The role of Care Coordinator is pivotal in the day-to-day running of our North West office. Working closely with the operational team, you will be responsible for the planning, coordination, and ongoing management of care packages, ensuring seamless service delivery to our valued clients.
You will be based at your local Apollo Home Healthcare office in Heywood, but travel across the North West region will be an essential part of this role. Therefore, we are seeking applicants who hold a Full UK Driving Licence and have access to their own vehicle. Candidates must be comfortable driving long distances and in different conditions, including motorway use.
- Effectively manage rotas for your care teams, responding efficiently to any day-to-day changes.
- Monitor care delivery to ensure it meets individual care plans and quality standards.
- Actively participate in multi-agency meetings with Integrated Care Boards (ICBs), advocating for client needs and contributing to care planning decisions.
- Support the out-of-hours on-call service on a 1 in 8 rota basis – with no requirement for hands-on care.
