We’re recruiting a care coordinator to manage rotas, support clients and families, and ensure smooth delivery of person-centred home care. Based in Heywood with travel across the Northwest. Full UK Driving Licence and own vehicle required.
Key Responsibilities:
- Client liaison
- Rota management and quick response to changes
- Monitoring care delivery
- Multi-agency meetings with ICBs
- Shared on-call support (no hands-on care)
