About the
Role: As a Complex Care Coordinator, you will play a vital role in ensuring our clients receive safe, high-quality care in their own homes.
Working closely with clients, families, healthcare professionals and care teams, you will coordinate complex care packages, manage staffing schedules, and ensure seamless service delivery across the Northwest region.
You will be based at your local Apollo Home Healthcare office in Heywood, but travel across the Northwest region will be an essential part of this role.
Therefore, we are seeking applicants who hold a Full UK Driving Licence and have access to their own vehicle.
Key responsibilities include
- Act as the first point of contact for our clients, their families, and healthcare professionals.
- Effectively manage rotas for your care teams, responding efficiently to any day-to-day changes
- Monitor care delivery to ensure it meets individual care plans and quality standards.
- Actively participate in multi-agency meetings with Integrated Care Boards (ICBs), advocating for client needs and contributing to care planning decisions.
- Support the out-of-hours, on-call service – with no requirement for hands-on care. What You will Need
- A Full UK Driving Licence and access to your own vehicle, plus confidence to travel across the Northwest region
- Previous experience in care coordination, rota management, workforce planning, field supervision, care management, recruitment coordination, or a similar operational role within health or social care.
- Understanding of person-centred care and planning.
- Strong organisational skills, with the ability to manage multiple care packages and respond quickly to changing priorities.
- Confident in making decisions with the ability to think on your feet and solve problems while staying calm and professional.
- Clear communication skills, with the ability to adapt your style to suit different audiences, including families, professionals, and external partners.
- Comfortable negotiating and influencing, particularly when managing rotas or balancing client needs.
- A team player with a positive, proactive approach and the ability to build strong working relationships.
