Aplos Health PCN in London is seeking experienced Mental Health Coaches to provide strategic support across the PCN. You will work with providers to ensure cohesive systems and processes, assess patients with low–medium need, and offer brief interventions or signpost to the right services.
You will collaborate with the MDT and a health & wellbeing coach, possibly extending services to adolescents aged 11–19 with low mental health needs. DBS checks are required and confidentiality is essential.
