As part of the reablement team you will have the reward of helping people as they leave hospital, supporting their rehabilitation following an illness or surgery. You'll play a huge part in their recovery and enable them to become independent again and ensure that they receive the best care possible, and emotional support they need in an undoubtedly stressful time.
