The Community Pathway Officer role will assist individuals experiencing mental health issues to stay in the community and prevent the need for a hospital admission or re-admission. The officer will provide an intensive (up to six weeks) outreach service to individuals who are in the crisis pathway, working in collaboration with the Norfolk & Suffolk Foundation Trust (NSFT). The service will support individuals to re-integrate into the community.

A full driving license and access to a vehicle are essential.
