Angela Hamlin Health Care Professionals are currently looking for a Client Liaison Nurse to join our growing team. We'd love to welcome a warm, experienced, and dedicated Registered Nurse to our team as Client Liaison Nurse. Key responsibilities:

  • Visiting clients in the comfort of their own homes to carry out thorough clinical assessments
  • Creating and keeping up-to-date personalised care plans that truly reflect each client's needs
  • Being a friendly, trusted point of contact for clients and their families throughout their care journey
  • Building strong relationships with GPs, consultants, and NHS teams to ensure joined-up care
  • Playing an active role in maintaining our CQC standards through audits, incident reporting, and safeguarding

What you'll need:

  • Full, current NMC registration as a Registered Nurse
  • Excellent clinical judgement paired with warm, confident communication skills
  • The ability to manage a varied caseload, working both independently and as part of a close-knit team
  • A naturally discreet, professional, and empathetic approach to client care
  • Up-to-date mandatory training
  • Enhanced DBS check, satisfactory references, and Occupational Health clearance
  • Computer skills - Microsoft

It would be great if you also have:

  • Experience caring for clients in their own home environment
  • A good understanding of CQC regulations and the Fundamental Standards
