The Blaenau Gwent Community Resource Team are seeking an exceptional Assistant Practitioner to join our dynamic integrated team. We are searching for individuals who are self‑driven, passionate and eager to make a real difference to people's lives. As part of our team, you will have the unique opportunity to help promote independence and empower individuals to achieve their personal goals. The Community Resource Team is a collaborative service between Aneurin Bevan University Health Board and Blaenau Gwent Social Services, dedicated to providing individuals with the right intervention at the right time, from the right professional(s). As an Assistant Practitioner, you will have the chance to make a real difference in the lives of adults in need of support to remain in their own homes. You will be an integral part of the wider multi‑disciplinary team comprising of physiotherapists, occupational therapists, health and well‑being support workers, social workers, medics and nursing staff.

Responsibilities

  • Work to enable service users to be supported in their own home wherever possible, by preventing hospital admissions and facilitating timely discharge from acute and community hospitals.
  • Assess and deliver treatment programmes, using good problem‑solving, time‑management and assessment skills to meet individual goals and outcomes.
  • Carry out independent caseload management within a broad framework supervised by Health and Care Professions Council registered professionals.
  • Respect service users' privacy, dignity, choice and diversity during all interactions.
  • Collaborate with the Rapid Response Team, Community Hospitals, Acute Hospitals, Home Care and Care Agencies.

The service is provided 7 days per week, between 7:30 am and 8 pm, 365 days per year. Staff must work across weekends and bank holidays on a rota basis.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

The Agenda for Change Pay Award for 2026/27 is effective from 1st April 2026. Successful applicants with a start date from 1st April onwards will receive a salary as per the 2026/27 pay award. Further details are available via the pay circular.

Education / Qualifications / Training

Essential Criteria

  • QCF/NVQ/GNVQ Level 3 or equivalent in health and/or social care plus relevant courses/training to support clinical experience and significant knowledge of the model of physiotherapy and occupational therapy with a particular emphasis on Reablement.
  • Working knowledge of Microsoft packages.
  • Knowledge and experience of using therapeutic equipment, including aids and adaptations.
  • Experience as a therapy assistant/technician, rehabilitation assistant or other equivalent post in an enabling role.

Desirable Criteria

  • Manual handling experience.
  • Evidence of continuing professional development.
  • Awareness of clinical governance.
