Senior Nurse - Primary Care Diabetes

An exciting opportunity has arisen for a Senior Nurse for Primary Care and Community Diabetes. With a diverse work programme, we are looking for a motivated, passionate and innovative registered nurse who is able to drive forward our ambitions for excellent Diabetes care and for improving patient experience, education and quality.

Main duties of the job

As a Professional Manager Senior Nurse and Clinical Leader, you will provide Clinical leadership for nursing within the Primary and Community Directorate across primary care and community services.

To support the Primary and Community Divisional Nurse, Head of Nursing Deputy Medical Director/Clinical Director and Directorate Manager in service development, operational clinical management and clinical governance arrangements for the delivery of direct nursing care by service teams. Provide Clinical leadership for the continuous professional development of Health Care Support Workers and the registered nursing staff within the Directorate. To lead on the delivery of integrated diabetes patient care across Primary, Community and Secondary Care services with very close links to the NCN's and to be operationally responsible for the management, training and development of Diabetes Specialist Nurses.

To work closely with Divisional and Senior Nurses, Consultants, Clinical Directors, GP's and primary, secondary care nurses across ABUHB to devise and implement education programmes for professionals involved with diabetes care on hospital wards, in general practice and in the community, and participate in other developments in this area as required. Support operational line-management of Primary Care and Community Service nurses and those that are directly employed within ABUHB Managed GP practice ensuring the delivery of quality driven, autonomous nursing practice.

Qualifications And Or Knowledge

Essential criteria

• Educated to Masters Level or demonstrable equivalent
• Knowledge of Diabetes in Primary Care and Community
• Specialist knowledge gained through additional training

Experience

• Experience of working at a senior level across the organisation to drive complex agendas
• Sound knowledge of patient experience quality outcomes
• Experience of interagency working and the development of collaborative and effective partnerships
