An exciting opportunity has arisen for a Senior Clinical Fellow to join our Community Care of the Elderly team based in Torfaen. As a team member, you will provide senior clinical input to our community wards, working closely with the wider multidisciplinary team to deliver safe, person-centred care for older adults. The role offers hands-on clinical experience across the community inpatient setting in County Hospital, supporting management of the older adult and discharge planning and collaborative decision-making with nursing, therapy, pharmacy and social care colleagues. This post is ideal for a doctor with experience in elderly care or general medicine who is keen to develop their skills in community geriatrics. It provides the opportunity to join a supportive team and gain exposure to innovative models of integrated care.
Main duties of the job
- The successful candidate will be a valuable and senior member of the Community COTE Directorate and will report to the senior physician.
- The role includes participating in outpatient clinics, attending multidisciplinary meetings, and performing independent ward rounds with senior support available where needed.
- The Senior Clinical Fellow will provide active medical input to patients admitted to the ward for rehabilitation, stroke rehabilitation or palliative care.
- Oak Ward has 29 beds across two floors; the Fellow will mainly be based on the Upper Oak Ward but will also support the lower floor as required, working closely with the Torfaen Community Resource Team and nursing staff.

Job responsibilities
- Assessment, monitoring and initiation of treatment with colleagues and under the supervision of the senior clinician.
- Attending daily board rounds and regular ward MDTs.
- Providing care for patients attending outpatient clinics.
- Attending multidisciplinary ward rounds as directed by the senior clinician.
- Completing discharge summaries within 24 hours of discharge of all patients.
The post holder will generally undertake programmed activities at their principal place of work or other locations as mutually agreed. Travel between sites and official meetings may be required. The post holder may be required to work at any site within the Health Board, including new sites, depending on service need.
This post requires a Disclosure to be made to the Disclosure and Barring Service to check for any prior convictions under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
£47,946 to £74,328 a year, pro rata for part time.
