Job Overview
Supporting the delivery of community-based diabetes education, this role helps individuals develop the skills and confidence to self‑manage their condition. Working alongside a multidisciplinary team, you will deliver engaging group and one‑to‑one sessions, promote healthier lifestyle choices, and improve access to education across local communities.
Main Duties
- Deliver diabetes education sessions (group and one‑to‑one) within community settings.
- Support individuals to develop self‑management skills and make informed lifestyle choices.
- Promote and deliver structured programmes such as DESMOND and other education modules.
- Manage referrals, triage patients, and coordinate education sessions.
- Monitor, record, and report on patient outcomes and programme effectiveness.
- Signpost patients to appropriate health, social, and community services.
- Work collaboratively with multidisciplinary teams and community partners.
- Contribute to service development, quality improvement, and patient‑centred care.
- Support training and awareness of diabetes education across community services.
