An exciting opportunity has arisen for a dynamic and enthusiastic individual to join the Mental Health & Learning Disabilities Commissioning Team within Aneurin Bevan University Health Board as Case Manager. The MHLD Commissioning Team oversees the arrangements for individuals who are commissioned packages of care. The post is a fixed term/secondment for 9 months due to secondment cover. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
Main duties of the job:
- The post holder will be responsible for the provision of seamless, integrated services and packages of care for individuals who meet Continuing NHS Healthcare criteria.
- They will play a key role in the implementation of the National Framework for Continuing NHS Healthcare within the Mental Health & Learning Disabilities Division and will provide a case management function to a caseload of patients/service users eligible for Continuing NHS Healthcare or who are subject to 117 and require commissioned services with needs that cannot be met by core services.
- The post holder will support the development and implementation of governance arrangements to improve the quality of care/service delivered to the local population, regardless of setting, and apply financial due diligence when commissioning packages of care.
