Primary and Community Services are at the heart of the model and central to developing a new relationship with patients, investing and strengthening community and social care services to support and treat patients in their homes and communities providing quality outcomes.

In order to deliver the strategy, an exciting opportunity has arisen for a motivated District Nursing Team Leader to join the Abergavenny District Nursing Team as part of a job share arrangement. This role offers the chance to work within a well‑established, supportive, and dynamic team dedicated to delivering high‑quality nursing care to patients within the community. We are looking to appoint someone who is passionate about providing high quality care and are committed to developing and investing in our staff to improve patient outcomes, while also supporting staff well‑being.

Responsibilities

  • The post holder will ensure the provision of cost effective, efficient and appropriate high quality patient care.
  • They will carry the continuing responsibility for the assessment, planning, implementation and evaluation of care needs and quality assurance.
  • The community nurse team leader will line‑manage a nursing team ensuring their professional and clinical development, promote evidence‑based practice and act as an accountable role model.
  • They will ensure that together with their team members they act in accordance with the NMC Professional Code of Conduct and other relevant policies.

The ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply.
