An exciting opportunity for a Discharge Coordinator on D3E, an elective surgical ward at Royal Gwent Hospital. The post holder facilitates safe and timely discharges for all patients within acute beds, working independently when required.

Main Duties

  • Gather information around patients’ pre‑admission ability, support mechanisms and concerns about post‑discharge services
  • Collect, record, collate, and report patient flow information as directed by manager
  • Support the ward to facilitate timely discharges
  • Actively contribute to daily capacity planning by managing and disseminating information on discharges and bed states
  • Liaise with ward managers, patient flow team and a range of clinical, social, and ambulance staff to progress discharges
  • Coordinate with care homes to reassess patients prior to discharge in accordance with CSSIW regulations
  • Finalize transfer arrangements to home or usual residence in a timely manner
  • Ensure all relevant patient information is obtained, stored and communicated, maintaining confidentiality at all times
  • Handle sensitive clinical and personal information with tact and diplomacy
  • Refer patients to third‑sector voluntary agencies following qualified staff direction to effect a safe discharge
  • Review information on PSAG boards and update accordingly

Qualifications and Knowledge

  • NVQ/QCF Level 3 or equivalent diploma level
  • IQT Bronze
  • Short courses or relevant experience
  • Evidence of up‑to‑date mandatory training

Experience

  • Experience working in an acute hospital
  • Active participation in complex decision‑making processes
  • Experience of direct patient care and/or working within a clinical setting
  • Experience of working where own decision‑making skills and judgement were utilised to solve problems
  • Experience of data collection and audit (desirable)

Aptitude and Abilities

  • Able to organise own workload and adapt to rapid changes and unpredictable workload
  • Exercise judgement when dealing with enquiries; analyse and resolve flow problems
  • Recognise when to escalate to line manager
  • IT literate in Word and Excel and good keyboard skills
  • Understands confidentiality and its relationship across settings and data
  • Provides non‑patient advice about patient discharge
  • Ability to speak Welsh (desirable)
