The salary scale for a Band 3 has been adjusted as part of the NHS Agenda for Change pay award for 2026/2027 and will be implemented in August 2026 backdated to 01 April 2026 where applicable.

The main duties of the posts are to assess and review individuals with complex health care needs within the community and hospitals. A vacancy has arisen with the Complex Care Team for an experienced Registered Nurse (RGN, RMN or RNLD) to undertake the role of Complex Care Nurse Assessor, based in Ty Gwent, Cwmbran.

The role will require the post-holder to undertake in-depth expert/specialist nursing assessments in hospital sites across the five Locality areas of Gwent, on behalf of Aneurin Bevan University Health Board, to ascertain individual’s eligibility for Continuing NHS Healthcare (CHC), Discharge to Assess and entitlement to NHS Funded Nursing Care (FNC) funding. Consequently, the role will require the post-holder to facilitate the Multi-disciplinary Team assessment process and to provide input into the discharge process.

The post-holders will promote collaborative and inter-agency working across professional and organisational boundaries by providing professional advice, support, and guidance to both professional and non-professional colleagues, patients, and their relatives/representatives. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Qualifications

  • Essential criteria: Degree level education
  • Adult Safeguarding level 1 & 2
  • Desirable criteria: Post graduate certificate or degree in Community care/nursing or relevant area of nursing. Short courses attended or postgraduate certificates in managing challenging behaviour, dementia.

Experience

  • Essential criteria: Experience of undertaking needs-based assessments and care plans, undertaking capacity assessments, gaining formal consent and the application of DoLS and the Mental Health Act.
  • Desirable criteria: Community nursing, knowledge of the Mental Health and Mental Capacity Acts.

Knowledge

  • Essential criteria: Dealing with cases with complex presentations, report writing, knowledge of Continuing Care legislation and policy.
