Job Overview

We are recruiting a permanent part‑time High Impact Service Specialist Nurse to support individuals who have frequent and/or high impact attendances within urgent and emergency care. The role involves building trusted relationships, delivering person‑centred care, and improving health outcomes while reducing inappropriate contacts with healthcare services.

Main duties of the job

  • Provide a High Impact Service within the Board, aiming to prevent unnecessary admissions, reduce length of stay, produce care and management plans, and collaborate with multi‑agency partners.
  • Assess patients with complex needs and high‑impact use of healthcare, including risk assessment and issues around substance use, social and psychological factors.
  • Work with other disciplines in the assessment, planning, implementation and evaluation of healthcare service provision to ensure a smooth patient pathway.
  • Provide consultation to relevant Staff in the Management of high‑impact service users and act as an accountable role model.
  • Participate in the development and implementation of healthcare policies and procedures to improve the patient journey.
  • Support the Clinical Lead in collating attendance data and presenting interpretations at meetings.
  • Represent the Urgent and Emergency Care viewpoint at operational and strategic meetings when necessary.

Experience

Essential criteria

  • Experience working with socially vulnerable groups.
  • Knowledge and understanding of urgent care processes and procedures.

Desirable criteria

  • Evidence of working within a multidisciplinary team.
  • Experience of case management.

Qualifications

Essential criteria

  • Current NMC registration.
  • Registered Nurse.

Desirable criteria

  • Attained or willingness to undertake Level 3 Safeguarding in Adults and Paediatrics.
