Aneurin Bevan University Health Board is seeking a Band 4 Assistant Practitioner to join the Blaenau Gwent Community Resource Team. In this role, you will work with a multi-disciplinary team to support adults, promote independence, and facilitate their well-being in a home setting.

Ideal candidates will possess a QCF/NVQ Level 3 in health/soc care, excellent communication skills and relevant experience. The position requires shift work across weekends and holidays, with a strong emphasis on teamwork and adaptability.
