In this role, you'll provide person‑centred therapeutic support to older people experiencing mental health difficulties across community settings, care homes, and memory assessment services. You'll carry out specialist assessments, reviews, and interventions for people with complex needs, including dementia, cognitive impairment and other mental health conditions, ensuring care is effective, evidence‑based and as least restrictive as possible.
Main Duties
- Work flexibly across services, supporting patients, carers and staff while also contributing to service development and quality improvement.
- Plan and oversee care for patients in both primary and secondary care, including those living in the community and in care homes.
- Conduct specialist mental‑health assessments, including memory and functional assessments, and manage a varied caseload across community and care‑home settings.
- Deliver practical, evidence‑based care for people with dementia and other mental health conditions, including care planning and regular review.
- Support timely diagnosis, manage risk and respond appropriately to crises.
- Collaborate with GPs, occupational therapists, psychologists and social care colleagues to ensure care is coordinated and effective.
- Maintain professional standards and ongoing development, support junior staff, contribute to a positive team culture and participate in service improvement programmes.
- Work within key legal frameworks, ensuring safe practice, safeguarding, accurate record‑keeping and confidentiality at all times.

Qualifications
- NMC registration (essential)
- Qualified for at least 1 year and completed a preceptorship programme (desirable)
