An exciting opportunity has arisen for a dynamic and innovative pharmacist to join the pharmacy team at Nevill Hall Hospital (NHH) as a Clinical Pharmacist with Neurology Directorate.As a key member of the multidisciplinary team, you will lead and deliver a high-quality, patient-centred pharmacy service within the Neurology Directorate. You will work closely with the Lead Pharmacist for Clinical Specialties and other healthcare professionals to ensure safe, effective, and evidence-based use of medicines.This role is ideal for a pharmacist with strong clinical knowledge, excellent communication skills, and a passion for service development. You will be responsible for providing expert pharmaceutical advice, supporting ward-based services, and contributing to the safe discharge of patients. You will also play a pivotal role in monitoring drug expenditure and supporting the implementation of prescribing guidelines.At NHH, we are committed to the professional development of our pharmacy workforce. Main duties of the job
Deliver specialist clinical pharmacy and ward-based medicines management services, including prescription review, resolving drug-related issues, and promoting evidence-based prescribing. Provide patient-focused care through bedside counselling, discharge planning, and medication reconciliation, ensuring safe transitions with patients, carers, and healthcare teams.
Act as the Directorate Pharmacist for the Neurology directorate, monitoring high-cost drug use, producing reports, supporting policy development, and contributing expert input at directorate meetings. As an independent prescriber, manage and prescribe medicines within your clinical scope.
Support education and training by supervising junior pharmacists, trainees, and technicians, and delivering sessions to healthcare professionals. Provide dispensary cover, including prescription checking and supporting automated systems like Omnicell and RDS.
Contribute to service development using quality improvement methods to enhance care and efficiency. Ensure governance and safety by reporting adverse drug reactions, medication errors, and maintaining compliance with legal and professional standards.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
