An exciting opportunity has arisen within the Caerphilly adult mental health service to appoint a community psychiatric nurse in the south Caerphilly area. The post holder is a senior member of the south Caerphilly mental health team and will be responsible for the care coordination of a caseload of service users experiencing severe mental health difficulties and additional complex care needs. This is a permanent post.
ResponsibilitiesThe successful candidate will work collaboratively across the wide-ranging multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area. The post holder will also support the delivery of timely and effective intervention for all individuals who need urgent mental health assessment and management.
The post holder will develop close working relationships with other aspects of the service, both community and inpatient units, and develop a broad knowledge of the resources available across the Borough. They will support the Team Leader to achieve organisational targets and demonstrate excellent communication and interpersonal skills. They will have a proven track record of working with service users with complex mental health needs and a commitment to providing person‑centred care. Ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
