In this role, you’ll provide person-centred therapeutic support to older people experiencing mental health difficulties across community settings, care homes, and memory assessment services. You’ll carry out specialist assessments, reviews, and interventions for people with complex needs, including dementia, cognitive impairment, and other mental health conditions, making sure care is effective, evidence-based, and as least restrictive as possible. You’ll work flexibly across services—supporting patients, carers, and staff while also getting involved in service development and quality improvement. You’ll also help plan and oversee care for patients in both primary and secondary care, including those living in the community and in care homes.

Main duties of the job:

• Working closely with older people and their families to provide compassionate, person-centred support, including having sensitive conversations around diagnosis and care needs in a reassuring and respectful way.
• Carrying out specialist mental health assessments, including memory and functional assessments, and managing a varied caseload across community and care home settings.
• Delivering practical, evidence-based care for people with dementia and other mental health conditions, including care planning and regular review.
• Supporting timely diagnosis, managing risk, and responding appropriately to crises.
• Collaborating with GPs, OTs, psychologists, and social care colleagues to ensure care is coordinated and effective.
• Maintaining professional standards and ongoing development, supporting junior staff, and contributing to a positive team culture.
• Being involved in service improvement, sharing knowledge, and helping to develop high-quality dementia care.
• Working within key legal frameworks, ensuring safe practice, safeguarding, accurate record-keeping, and maintaining confidentiality at all times.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

• Knowledge of Medications used in our practice

Qualifications:

• NMC Registration

Desirable criteria:

• Sufficient experience at least having completed preceptorship program
