Responsibilities
- Work as a member of the Colorectal multi-disciplinary team (MDT) contributing to the delivery of a specialist service working alongside and supporting the Consultants.
- Act as an autonomous practitioner, interpreting, communicating and acting on findings, referring patients for further investigations and/or treatment where appropriate.
- Act as a lead specialist for the area.
- Co-ordinate, set‐up and run nurse‐led follow‐up clinics to support the Consultant pool.
- Independently run a weekly nurse‐led new patient clinic to support the Consultant in management of patients on a suspected cancer pathway.
- Participate in the running of virtual follow‐up clinics as according to service need.
- Evaluate patient pathway outcomes and make decisions about care plans.
- Oversee and manage watch‐lists of patients, providing assurance that patients presenting to the Colorectal CNS are receiving optimal care, escalating where necessary.
- Promote and ensure effective communication within multidisciplinary teams across ABUHB, developing close partnerships and productive working relationships with key individuals and groups.
- Work towards being a registered non‐medical independent prescriber.
- Promote and support the rights, responsibilities and diversity of patients and their families/carers with kindness and empathy.
- Use appropriate advice/alert agencies when there are any concerns regarding potential/actual risk to any patient or their dependents.
- Ensure that patient records are maintained, are up to date and compliant with the Data Protection Act, actively promoting the use of e‐documentation.
- Be accountable for your own professional actions, adhering to the standards in the NMC Code of Professional Practice (2018).
- Delegate the delivery of care where appropriate in line with NHS Wales delegation framework and the NMC Code of Practice (2018).
