Job Overview
An exciting opportunity has arisen for Band 4 Assistant Therapy Practitioners to join our Community Admission Avoidance Therapy Team (CAATT) at the Royal Gwent Hospital, working Monday to Friday. The post involves supporting people who present to emergency or acute medical units, helping prevent unnecessary hospital admissions, reducing hospital stay length, and providing care closer to home by liaising with primary, community, and third-sector organizations.
Main Duties
This role is integral to our multi-professional service delivery. The successful candidate will deliver delegated therapeutic interventions, liaise with patients, families, MDT members, and external agencies, and manage a caseload of patients with various functional difficulties, including those with co-morbidities, frailty, and cognitive deficits. Good time management, prioritization, flexibility, and effective communication skills are essential. Collaboration with registered Occupational Therapists, Physiotherapists, and Dietitians will be routine. Welsh language skills are desirable; applications from Welsh and/or English speakers are equally welcome.
