An exciting opportunity has arisen for a Band 4 Assistant Practitioner to join the Abergavenny District Nursing Team on a permanent contract for 30 hours. The Abergavenny team provide clinical nursing care and interventions for patients in their own home under the medical governance of Primary Care GP's. The District Nursing/Community team in Abergavenny works as a partner within a wider multi-disciplinary team which includes Frailty Consultant, General Practitioners, Care at home, Physiotherapists, Occupational Therapists, Social Workers, Support and Wellbeing workers and the Voluntary Sector.

Please note applicants must hold or be working towards Level 4 vocational qualification in a relevant health care associated subject / Certificate in Health Care Support Worker Education. An informal visit by anyone prior to application would be welcomed by the team.

Main duties of the job

Are you motivated, enthusiastic and want to progress in your career? As Assistant Practitioner you will work with the wider community MDT under the delegation of your Band 7, Band 6 and a team of Registered Nurses. You will utilise and develop a wide variety of skills to meet the emotional, physiological, and clinical needs of individuals. The post holder will work collaboratively and support members of the multidisciplinary team in the delivery, development and management of health and appropriate referral for social care issues. The role is challenging but rewarding and you will be fully encouraged and supported with your professional development. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. This post is permanent and part time.

Person Specification

  • Experienced in undertaking a range of clinical skills relevant to the post as per role scope of practice and the national governance framework
  • Significant and recent experience of providing relevant clinical care
  • For community roles: experience of providing care in a community or primary care setting.
  • Level 4 vocational qualification in a relevant health care associated subject / Certificate in Health Care Support Worker Education
  • GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
  • Ability to speak or learn Welsh
  • Trained to perform venepuncture
  • Interpersonal skills and clinical skills
  • Developing new services, ability to speak Welsh
  • Values: care, compassion, communication, competence, courage and commitment
