This is a full-time position for a licensed social worker. The Medical Case Manager provides education, counseling, cognitive-behavioural interventions and problem-solving to a highly receptive outpatient population of sickle cell patients and their families in an environment that values social work services.
Responsibilities:
- Provide supportive counseling sensitive to understanding the impact of sickle cell, body changes, financial impact, social difficulties, financial resources, complicated coping.
- Be visible and accessible; attend multidisciplinary meetings, participate on appropriate Sickle Cell and community committees.
- Provide a forum for those with varied needs to obtain support.
- Create meaningful quality of life experience for those facing life threatening crises, retreats, use complimentary therapy.
- Assist the Sickle Cell Clinic staff with staying energized.
- Participate in fundraising for new and existing projects.
Qualifications:
- Current, unencumbered licensure as a Licensed Baccalaureate Social Worker (LBSW).
- Two years of social work experience preferred.
- Experience with the MS Office Suite of products, including Excel.

- Eager to learn more about the social and medical support requirements of our practice population.
