Summary
A Care Team Leader is responsible for leading and supporting the care team to deliver high-quality, person-centred care for residents living with dementia.
Key Responsibilities:
- Lead and support care staff during shifts.
- Deliver and oversee person-centred dementia care.
- Administer and monitor medication in line with policies.
- Maintain accurate care records and reports.
- Support residents with daily living activities and emotional wellbeing.
- Liaise with families and healthcare professionals.
- Conduct handovers and escalated concerns appropriately.
- Ensure health and safety, safeguarding, and regulatory standards are met.
- Mentor, supervise, and support the development of care staff.
