Amplius is looking for a Reserve Community Outreach Support Worker to help people with enduring mental health conditions live more independently. This flexible, relief role requires you to work on an as-needed basis across a 8:30am-8:30pm window, including evenings and weekends, in Peterborough.
You will support customers with practical tasks, create and review support plans, carry out risk assessments and coordinate with mental health teams to promote wellbeing and tenancy sustainability.
