SALARY - $120K - $180K per year (depending on experience)

Responsibilities

  • Provide comprehensive physiatry evaluations and ongoing rehabilitation management in skilled nursing and long-term care settings.
  • Develop individualized treatment plans focused on functional restoration and improved patient outcomes.
  • Collaborate with therapy teams, nursing staff, and facility leadership to support rehab goals and continuity of care.
  • Manage musculoskeletal, neurologic, orthopedic, and medically complex cases within the post-acute population.
  • Perform procedures as appropriate (joint injections, spasticity management, etc.).
  • Document patient encounters in accordance with regulatory and company standards.
  • Support quality initiatives and maintain clinical excellence across assigned facilities.

Qualifications

  • MD or DO from an accredited medical school.
  • Board Certified or Board Eligible in Physical Medicine & Rehabilitation.
  • Active Pennsylvania medical license (required or ability to obtain).
  • Experience working in long-term care, subacute rehab, or post-acute environments strongly preferred.
  • Strong communication, leadership, and clinical decision-making skills.
  • Ability to work independently across contracted facilities.
