Responsibilities
- Provide comprehensive physiatry evaluations and ongoing rehabilitation management in skilled nursing and long-term care settings.
- Develop individualized treatment plans focused on functional restoration and improved patient outcomes.
- Collaborate with therapy teams, nursing staff, and facility leadership to support rehab goals and continuity of care.
- Manage musculoskeletal, neurologic, orthopedic, and medically complex cases within the post-acute population.
- Perform procedures as appropriate (joint injections, spasticity management, etc.).
- Document patient encounters in accordance with regulatory and company standards.
- Support quality initiatives and maintain clinical excellence across assigned facilities.
Qualifications
- MD or DO from an accredited medical school.
- Board Certified or Board Eligible in Physical Medicine & Rehabilitation.
- Active Pennsylvania medical license (required or ability to obtain).
- Experience working in long-term care, subacute rehab, or post-acute environments strongly preferred.
- Strong communication, leadership, and clinical decision-making skills.
- Ability to work independently across contracted facilities.
