About The Role
We have an exciting opportunity to join our talented team as a Dementia Adviser. You will have the rewarding experience of providing information, guidance, and support to people affected by dementia in West Somerset.
You will:
- Primarily work across the West Somerset area, with the ability to cover other areas of Somerset as required.
- Assist people with dementia and their carers in identifying their needs, providing information and guidance to help maintain their independence, improve their well-being, and empower them to take control of their lives.
- Build relationships with local contacts, network with health and care professionals, and provide dementia support.
- Develop networks and pathways with partner organizations working in dementia care to ensure seamless support and empower individuals to make informed choices.
- Provide support face-to-face, by telephone, and online, maintaining detailed and confidential records.
About you
We are seeking a highly motivated individual with an understanding of dementia and the needs of those living with or caring for affected individuals. You should be approachable, knowledgeable, and able to join our experienced team.
Our Dementia Advisers provide person-centered support in homes, face-to-face, via telephone, and virtually, offering tools, knowledge, and strategies for living well and making informed decisions. As a community-based worker, you will attend meetings, appointments, and work from various venues, including home.
Interviews will take place the week commencing 4th August.
You will:
- Be empathetic and non-judgmental, with a commitment to equal opportunity.
- Ideally have knowledge of Adult and Child Safeguarding, Information Governance, and laws like the Mental Capacity Act.
- Possess good communication skills to meet diverse community needs and liaise with statutory and voluntary agencies.
- Have a sound knowledge of IT systems for data recording, reporting, and communication.
- Understand client confidentiality.
- Preferably have experience working with relevant agencies.
- Be able to travel independently around Somerset.
