We are looking for an enthusiastic, self-motivated and empathetic individual to join our Social Prescribing team. Social prescribers help to reduce health inequalities by supporting people to unpick complex issues affecting their wellbeing and enabling them to have more control over their lives. Through creating personalised health and wellbeing plans, goals are set to help people overcome barriers they face, by connecting them with local VCSE organisations, community, and neighbourhood groups.

Ideally you will have a good knowledge of health and social care and voluntary services in the area and be keen to take on a new challenge as a Social Prescribing Link Worker within our successful team. You will be person-centred, dedicated and committed to making a positive difference to the lives of our patients. Working in a supportive patient-facing role, you will build strong relationships with the voluntary, community and social enterprise sector. As our Social Prescribing Link Worker, you would also work with a wider team of social prescribers throughout Trafford and Greater Manchester and be integral to future plans of the VCSE sector and health and social care in the area.

Main duties of the job

  • Promote social prescribing, its role in self-management, addressing health inequalities and the wider determinants of health.
  • As part of the PCN multi-disciplinary team, build relationships with staff in GP practices within the local PCN and Trafford PCNs, attending relevant MDT meetings, providing information and communicating feedback on social prescribing.
  • Meet people on a one-to-one basis. Give people time to tell their stories and focus on 'what matters to me'.
  • Help people identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities.
  • Work with individuals to co-produce a simple personalised support plan to address the person's health and wellbeing needs based on the person's priorities, interests, values, cultural and religious/faith needs and motivations.
  • Where appropriate, physically introduce people to culturally appropriate community groups, activities and statutory services, ensuring they are comfortable, feel valued and respected.
  • Forge strong links with a wide range of local VCSE organisations, community and neighbourhood level groups, utilising their networks and building on what’s already available to create a menu of diverse community groups and assets, who promote diversity and inclusion.

About us

Altrincham Healthcare Alliance PCN is a forward-thinking Primary Care Network consisting of five GP practices in south Trafford. We are committed to providing patient-centered healthcare by working together with community, mental health, social care and voluntary services in our area. You will work alongside GPs, Nurses, Physios, Pharmacists and other practice staff at each GP practice to support patients with Social Prescribing. All staff have access to the NHS pension scheme.
