As a Reablement Worker, you’ll work closely with our clients, their families, and other professionals to provide reablement support in people’s own homes helping them regain independence, confidence, and control over their daily lives. You’ll enable and encourage, not just do for supporting people to achieve their own goals and live as independently as possible. What You’ll Be Doing:

As a Reablement Worker, you’ll play a vital role in helping people regain their independence and confidence in daily living. You’ll work closely with individuals in their own homes, empowering them to make their own choices and supporting them to achieve their personal goals.

About The Role:

  • Work collaboratively with clients to set meaningful goals and identify the steps needed to achieve them.
  • Support and assist people in developing skills and confidence to live as independently as possible.
  • Work alongside individuals to rebuild skills and confidence in personal care and everyday activities, supporting independence with choice, dignity, and control.
  • Offer physical and emotional support, encouraging participation in daily living tasks and activities.
  • Monitor progress and report when individuals have achieved independence and are ready to move on.
  • Identify and respond promptly to any safeguarding concerns in line with established policies and procedures.
  • Build and maintain positive relationships with clients, their families, colleagues, and other professionals to ensure person-centred, safe care.
  • Maintain accurate and timely records using mobile technology to ensure quality care delivery and outcomes.
  • Involve the person you’re caring for by explaining your actions, like updating notes on the mobile care app.
  • Implement and uphold safe working practices in line with company values and standards.
