About The Role
As a Support Worker at AFG, you’ll support people to achieve their goals and live as independently as possible. This may involve working in people’s own homes, community-based services, adult care homes, or independent hospitals.
- Support individuals with diverse care needs (learning disabilities, physical disabilities, mental health concerns, complex care, substance misuse, etc.).
- Develop and implement personalised care plans focused on clients’ needs and aspirations.
- Provide assistance with daily living, activities of daily living, and community integration.
- Monitor and record client progress and report to team as required.
- Collaborate with multidisciplinary team and families to deliver person‑centred care.
