Provide complex care and support to new individuals, ensuring all paperwork is in place before initial care delivery, and shadow-train new support workers. Travel around the North West as required.
Responsibilities
- Travel around the North West to provide short-term care and support to new individuals in accordance with their Personalised Plan.
- Shadow-train new core care team support workers, ensuring competence for clinical nurse sign-off.
- Ensure all necessary paperwork is in situ within the home setting prior to initial care delivery.
- Assist with initial rostering and coordinating rotas for the core care team.
- Act as a role model, consistently demonstrating AFG’s values and safe, effective, high-quality care.
- Provide health-related and social support to people with complex care diagnoses (e.g., Acquired Brain Injury, Spinal Injury, Learning Disability, Autism, Mental Health).
- Support people who present behaviours that challenge, using appropriate techniques and strategies.
- Provide safe medication and pain management in line with the medication care plan.
- Manage medical equipment and supplies, monitoring depletion and re-ordering as necessary.
- Attend all clinical mandatory and specialist training sessions, both face-to-face and online, to remain competent.
- Report and record incidents which could cause harm in a timely manner, following AFG policies.
- Promote independence, community engagement, and person-centred planning, supporting people to make informed choices and exercise their cultural beliefs.
- Ensure people are not abused or neglected, reporting concerns to line management or via whistleblowing procedures.
Principal Accountabilities
- Provide personalised support with daily living activities, medication administration, and equipment management.
- Assist people to achieve outcomes defined in the Personalised Plan, ensuring safe, effective care.
- Support development and maintenance of relationships, as identified in the Individual’s Personalised Plan.
- Facilitate healthy choices, leisure, work, social, emotional, and spiritual needs.
- Connect people with community resources and signpost to other supports.
- Help manage finances for financial independence and stability.
- Maintain a clean and safe living environment and ensure home safety and security.
- Support health and wellbeing, including medication and personal care needs.
Training & Development
- Participate fully in the Alternative Futures Group development programme for Clinical Community Care Support Workers and any external training.
- Attend and actively participate in supervision, performance reviews, and team meetings.
- Develop and share skills to improve support provision.
Code of Conduct
All staff must uphold the Skills for Care Code of Conduct for Healthcare Support Workers and Adult Social Care Workers. You will be given the code before you begin work.

- Be involved in out-of-hours support as part of an on-call rota.

Essential Criteria
- At least 2 years experience providing clinical care and support services to adults with complex care needs (e.g., Acquired Brain Injury, Spinal Injury, Learning Disability, Autism, Mental Health).
- At least 2 years experience providing medication and pain management for people with complex care diagnoses.
- Experience of shadow-training new complex care workers.
- Experience of working with people from a wide variety of backgrounds.
Qualifications & Training
NVQ or QCF Level 2 in Care / Promoting Independence or equivalent.
Other Requirements
- Willingness to attend all clinical mandatory and specialist training courses and events.
- Ability to travel to services throughout the North West region for short-term care and support.
- Flexibility to work flexible hours, covering 24/7 service needs.
- Willingness to provide out-of-hours support and participate in an on-call rota.
- Full driving licence and access to a vehicle.
