We want to measure and evidence this Positive Impact through people’s personal goals and outcomes utilising our bespoke, co‑produced outcomes framework model.
A personal goal and corresponding outcome must be based on increasing independence, choice and control under the domains of Occupation, Relationships, Daily Living Skills, Travel and Health & Wellbeing.
Job Purpose
- To lead on the implementation of a strategic approach to goals & outcomes across the whole business, ensuring that the necessary systems, policies, procedures and support materials are developed and embedded.
- To lead on the cultural change required across Operations to embed the philosophy of independence, choice and control and active citizenship through personal goals and outcomes. This will include training, coaching, mentoring and signposting to resources.
- To partner with the Co‑Production Lead to assist with driving the wider co‑production agenda across the organisation. Thus, improving personal outcomes, user experience and organisational effectiveness.
- To lead on developments within our brand new Digital Support Record system to improve the recording, monitoring and reporting of goals and outcomes.
- To work on future developments for utilising goals & outcomes data to support a value model to evidence the benefit of AFG support to commissioners and other payors.
Dimensions
- There is currently no line management responsibility.
- Responsible for a small non‑pay budget circa £20k
Key Accountabilities And Responsibilities
- Leadership and delivery of AFG’s Goals & Outcomes Framework through engaging senior leaders, managers, staff, people supported, patients and carers.
- Developing organisational capacity and capability to understand and practice independence and active citizenship support models through personal goals and outcomes effectively by mentoring and coaching.
- Design and implement an approach to the evaluation of personal goals and outcomes and the impact on people supported, patients and carers and our wider charitable purpose.
- Develop and produce a monthly update report to the business on progress in goals and outcomes.
- Develop and produce an annual report to the AFG Board on progress in goals and outcomes.
- Work within the defined process and limits of AFGs Scheme of Reservation and Delegation (SORD).
- Managing and delivering within agreed targets the goals and outcomes non‑pay budget and annual business plan deliverables to ensure cost‑effective deployment, management and utilisation of resources and non‑pay spend.
- Maintain an understanding of principles of goals and outcomes, co‑production, person‑centred support and best practice guidance, as well as national developments participation and involvement in learning disabilities, autism and mental health services.
- Acting as a Leader and Ambassador for the organisation, its purpose and its people at all times. Demonstrating a high level of expertise and understanding of the AFG strategic ambition for quality, value and personal outcomes and effectively contributing to the overall strategic development of the organisation as a member of the Quality Team.
- Ensuring on‑going feedback on the organisation is obtained from individuals, families, professionals and groups to determine priorities for improvements to organisational objectives. Ensuring a culture of engagement and full involvement of the people we support.
- Developing connections with operational teams that actively encourage and increase the voices of people we support in the running of AFG and upholding the reputation of the organisation as a ‘person‑centred’ organisation.

Health & Safety
You have a legal responsibility not to endanger yourself, your fellow employees and others by your individual acts or omissions. The post holder is required to comply with the requirements of any policy or procedure issued in respect of minimizing the risk of injury or disease. All accidents must be reported to your manager and in line with the general philosophy of the organisation.
Skills & Knowledge
- Resilient and able to work at a pace and manage competing priorities.
- Excellent communication skills (both verbal and written)
- IT literate.
- An understanding and knowledge of good practice in goals & outcomes, co‑production, person‑centred planning, participation and involvement.
- An understanding and knowledge of health and social care policy, legislation and regulatory frameworks around outcomes, co‑production, person‑centred care, advocacy and participation.
- Knowledge of change management theory and practice
- Group facilitation and servicing of working groups
- Understanding and knowledge of accessible communication

Experience
- Experience of working with a wide range of organisational stakeholders
- Experience of working with senior managers including writing and presentation of reports.
- Experience of setting and monitoring co‑produced goals & outcomes with people with lived experience of mental health issues, learning disabilities and autism.
- Experience of leading and managing within complex systems
- Experience of facilitating and servicing service user and/or co‑production support groups or similar
- Experience of communicating complex information to a wide variety of stakeholders taking into consideration their access requirements
- Experience of understanding complex information and presenting it clearly and in various accessible formats to a range of different stakeholders
- Experience of convening, designing and delivering training
Qualifications And Training
- Demonstrable continuous professional development
Other
- Car driver with full licence.
- Ability to travel across the North West and wider.
- Expectation to be in operational localities at least 2 days per week
