My client is a health and social services organisation who service the South Auckland area. They are a fantastic team and their mission is to provide high quality, accessible and affordable healthcare to whānau.

You will work alongside whānau across Auckland to support them through their cancer journey, helping them understand their diagnosis, navigate the health system and access the services they need. This role is full time, Monday to Friday 8:30am to 5:00pm and you will be based in Mangere.

The role, in a nut shell:

  • Connecting with whānau referred to the Cancer Care Coordination Service
  • Supporting whānau to understand their diagnosis, treatment, and care pathways
  • Helping whānau attend appointments and move through treatment and recovery
  • Completing needs assessments and identifying supports required
  • Connecting and advocating for whānau with health, social and community services
  • Coordinating practical supports such as transport and accommodation
  • Supporting whānau to stay engaged with treatment and services
  • Maintaining case notes, data collection and reporting requirements
  • Building strong relationships with internal teams and external providers to support referral pathways

To be successful in this role:

  • Experience in health, community or social services
  • A good level of communication and relationship-building skills
  • Ability to engage effectively with whānau and community partners
  • Good organisational and computer skills, with the ability to learn new systems
  • Ability to manage a caseload and prioritise your workload
  • Emotional resilience and ability to support whānau through challenging situations
  • Supporting whānau to stay engaged with treatment and services
  • Full New Zealand Driver's Licence
  • Either permanent residency or NZ citizenship.
