This role helps young people with ongoing health needs move confidently from children’s to adult nursing services, making a real difference to their long‑term wellbeing. You’ll work across children’s and adult community nursing teams to make sure transition is planned, joined‑up and centred on each young person. The role focuses on coordination, advocacy and quality, helping young people and their families feel informed, supported and listened to at a time of change.

What you’ll do:

  • Coordinate smooth transitions for young people from around age 14 into adult services
  • Act as a key contact, tracking progress so no one is lost during transition
  • Chair transition multidisciplinary meetings and follow up agreed actions
  • Support young people and carers to understand choices, consent and capacity
  • Share skills and learning with colleagues to build confidence across teams

What we’re looking for:

  • A registered nurse with current Nursing and Midwifery Council registration
  • Experience supporting young people or young adults with complex health needs
  • Confidence working across teams and services, using good judgement
  • Strong communication skills, with a calm and inclusive approach
  • An interest in learning, teaching others and improving how services work
