The Referral Support Care Navigator will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients going through faster diagnosis referrals, vague systems and screening pathways where there is a suspected cancer.

Work as a Health/Social Care Assistant to assist with the coordination and management of patients undergoing diagnostic, screening fast track and treatment at The Royal Wolverhampton NHS Trust to support patients both on and off treatment.

  • Provide information to patients related to first appointments, treatment/follow-up and signpost to appropriate healthcare providers within remit of role and guidance.
  • Support the fast-track team to book appointments where service dictates.
  • Provide telephone advice and triage where appropriate, working within defined protocols and guidelines.
  • Document and communicate outcomes as per service specification.

Macmillan Cancer Support and Information Support Manager in developing and facilitating Health and Well Being Programmes. This will involve contributing and delivering education to patients and carers, including information around health promotion, disease prevention and supporting self-management of their rehabilitation or long-term condition. This could be across hospital sites and off site locations.

  • Ensure patients, family and carers are treated with dignity and respect and in keeping with the RWT approach and Macmillan values.
  • Provide dedicated and knowledgeable support to patients receiving care and treatment at RWT.
  • Book appointments and contribute to the delivery of the faster diagnosis target.
  • Help patients and relatives overcome barriers to understanding when English is not their first language or where accessible information is required.
  • Participation in the development of Well Being Sessions.
