An exciting opportunity has arisen to work alongside the Fundamentals of Care Lead Nurse/AHP/Midwife. You will play a key role in supporting the delivery of the Trust's Fundamentals of Care Strategy and the development of the new Fundamentals of Care Academy. This rewarding role will provide visible practice-based support across our clinical, community and outpatient services, helping teams to deliver safe, effective, compassionate and person-centred care.
You will support quality improvement initiatives, education, coaching, audit, assurance and the use of data to drive sustainable improvements in care standards and patient outcomes. We are looking for an enthusiastic and motivated individual with excellent communication and relationship-building skills, who can influence, inspire and support teams to embed best practice and reduce avoidable harm.

The post holder will support the Fundamentals of Care Lead Nurse/AHP/Midwife, Quality and Safety Lead and Corporate Director of Nursing to deliver and embed the Trust-wide Fundamentals of Care Strategy and Fundamentals of Care Academy. The Fundamentals of Care Improvement & Practice Facilitator will provide visible practice-based support, education, coaching and quality improvement facilitation across WWL, helping clinical teams to consistently deliver safe, effective, compassionate and person-centered care.

The Fundamentals of Care Improvement & Practice Facilitator will support the Band 7 Lead as an accessible point of contact for matters relating to fundamental care, developing and maintaining effective working relationships to support the delivery of high-quality, person-centred care.

Key Responsibilities:
- Contribute to improved quality, safety and consistency of fundamental care across all Trust services and sites.
- Support the implementation and embedding of the Trust’s Fundamentals of Care Strategy and Fundamentals of Care Academy.
- Contribute to the reduction of avoidable harm across harm-free care priorities, including but not limited to skin integrity, falls prevention, sepsis, nutrition and hydration, continence care, infection prevention and control, personalised care, patient experience, dignity, comfort, communication, documentation standards and risk assessment completion.
- Improved use of audit, data, evidence and quality improvement methodology to identify variation, support improvement activity and demonstrate measurable outcomes.
- Increased staff knowledge, confidence and capability in delivering the fundamentals of care through education, coaching, role modelling and practice-based support.
- Greater standardisation of evidence-based practice, tools, processes and documentation across clinical areas.
- Support strengthened governance and assurance through clear reporting, escalation of risks, monitoring of key performance indicators and evidence of learning from incidents, complaints, audits and patient feedback.
- Improved patient, family and carer experience through care that is compassionate, person-centred, dignified and responsive to individual needs.
- Effective collaboration with divisional, community, corporate and multidisciplinary teams to support sustainable improvements in care delivery.
- Support enhanced use of digital platforms, clinical systems, dashboards and reporting tools to enable improvement, assurance, standardisation and decision-making across the fundamentals of care agenda.
