Your role: As a Community Link Worker, you will support residents to improve their health, wellbeing and quality of life by connecting them with local services, activities and community support. Working closely with GP Practices, voluntary organisations and community groups, you will manage your own caseload, provide one-to-one support, and use coaching and motivational interviewing techniques to help people achieve positive outcomes. You will be part of the Be Well service, building strong relationships across neighbourhoods and empowering residents to become more independent and connected within their communities.

About you: We are looking for an empathetic, motivated and approachable individual with experience of supporting people on a one-to-one basis. You will have excellent communication and relationship-building skills, a good understanding of local communities and the barriers people face to improving their wellbeing. Experience of working with community groups, voluntary organisations, healthcare services or GP practices is desirable. You should be confident working independently, managing priorities, maintaining accurate records and using coaching or motivational techniques to empower residents to make positive changes.
